Role: Personal Wellbeing Navigator
Based: North Yorkshire
Salary: £24,000

Hours: 35 hours per week
Our client, a well-respected charity, dedicated to supporting people impacted exploitation, violence and the criminal justice system.
You would be supporting service users serving community sentences or being released from prison.
Synopsis of duties:
- Deliver person-centred, trauma-informed support to individuals referred by the Probation Service, including those serving community sentences or leaving prison.
- Support service users with emotional wellbeing, family and relationship issues, social isolation, and positive lifestyle/decision-making.
- Conduct assessments and create action plans to achieve timely, prescribed outcomes for service users.
- Provide “through the gate” support for individuals transitioning from prison to the community.
- Deliver individual and group interventions tailored to service user needs
- Work flexibly across a wide geographical area to meet service users in various locations.
- Build and maintain positive relationships with probation, prisons, and external partner agencies.
- Achieve contractual targets and outcomes within agreed timescales and quality standards.
- Record all caseload activity accurately in CRM systems and provide updates/reports as required.
- Maintain confidentiality and data security in line with organizational policies.
- Promote sustainability and reduce the environmental impact of work practices.
Essential Requirements:
- Experience working with male adults in the criminal justice system, in community or prison settings.
- Experience engaging with challenging individuals, including those with complex needs or reluctance to engage.
- Experience collaborating with partner agencies or multi-agency teams to achieve shared objectives.
- Level 3 in Advice and Guidance (preferred, not essential).
- Understanding of caseload management, record keeping, and meeting action plan goals and deadlines.
- Must drive with access to a car.
- Enhanced DBS on the update service or will to process.
- Must have EL2 vetting
