Supporting Futures are recruiting on behalf of a well-respected charity for a Young Carers Practitioner to support children, young people and young adult carers within Cumbria.
This is a rewarding opportunity to make a genuine difference to the lives of young people aged 5-25 who are impacted by caring responsibilities within their family. Working within Family Hubs and the local community, you will provide tailored one-to-one support, complete assessments, and help young carers access the services and support they need to thrive.
About the Role
You will manage a caseload of approximately 12 young people.
The role focuses on supporting children and young people who are caring for parents or family members with physical health conditions, mental health needs, emotional wellbeing challenges or disabilities. Through assessment, direct support and partnership working, you will help reduce inappropriate caring responsibilities and improve outcomes for young carers and their families.
As this is a relatively new service, we are particularly interested in candidates who can confidently build positive relationships with schools, community organisations and partner agencies to raise awareness of the service, strengthen referral pathways and support future service development.
Key Responsibilities
• Complete Assessments
• Manage a caseload of young carers and their families, providing tailored support and intervention
• Deliver one-to-one support sessions focused on resilience, wellbeing, confidence and coping strategies
• Develop bespoke support plans and monitor progress towards positive outcomes
• Work collaboratively with schools, health services, social care and partner agencies
• Attend multi-agency meetings
• Promote access to appropriate support services and community resources
• Maintain accurate records and manage safeguarding concerns appropriately
What We're Looking For
We are particularly keen to speak with candidates who can demonstrate:
• Experience completing assessments with children, young people and families, ideally including Young Carer Needs Assessments or similar holistic assessments
• Strong knowledge of safeguarding, child development and the challenges faced by children and young people
• Excellent communication and relationship-building skills
• Experience supporting children, young people and families within community, education, family support, youth work, early help or related services
• Ability to build positive working relationships with schools, partner agencies and stakeholders
• Strong organisational, case management and record-keeping skills
• Full UK driving licence and access to a vehicle
